Before coming in for your first appointment at the Marin Cancer Institute, it is a good idea to take the time to fill out any forms your doctor has asked you to complete ahead of time. By reviewing these forms in advance, you will be more prepared for your visit with your care team. These forms are designed to help your doctors learn as much as they can about you and your health history so they can give you a thorough evaluation of your condition.
Medical release forms allow your physician to retrieve past medical records from your primary care physician or other specialists you may have seen. Because the care team at the Marin Cancer Institute often works collaboratively with a multidisciplinary panel of specialists to provide superior care, medical release forms facilitate the communication among the physicians in the group.
